Documenting your Residential loan application You may access the Uniform Residential Loan Application by clicking here. You must have access to Adobe Acrobat Reader and a printer. If you do not have Adobe Acrobat Reader, you may click here to download the free version. Once you have accessed the Loan Application, you may type in your information by using the "Text Selection Too." Please sign the “1003” mortgage application & “ALL” disclosures. (please read these forms and call if you have any questions). PLEASE NOTE: THE APPLICATION FORM REQUIRES LEGAL PAPER FOR PRINTING. Copies of your most recent bank statements for two months, or quarterly statements on all asset accounts. Two months from today’s date backwards. (401K, IRA, PERS, any other to show assets) Please include “ALL” pages of each statement. Income - Pay stubs for the last 30 days from today’s date back, for all borrowers. Copies of your 2002 & 2003 W2 forms. ( If you do not yet have a W2 for 2003 please add your last pay stub from Decemeber 2003 documenting your year to date income for 2003. If self-employed, provide a Year-to-Date Profit and Loss Statement and most recent two years Federal Tax Returns on business entity, i.e. corporation, partnership, as well as K-1. Documentation of child support/alimony received if you are going to use it as a source of income. Copies of checks for the last 12 months will be required. Please include “ALL” pages of any documentation supplied. Complete copy of dissolution of marriage papers, if applicable. Please include “ALL” pages of any documentation supplied. Copy of rental agreement(s ) on any rental property you own if the income is used to qualify for loan. Original Certificate of eligibility (will be returned) and DD14 form for VA government loans. If applicable, complete copies of bankruptcy papers (including discharge and list of debtors). Please include “ALL” pages of schedules. If applicable letters of explanation regarding: Derogatory credit. (Form enclosed) Work history if you have any gap’s in employment in the last two years. Increase in housing expense. Any large deposits. If you are NOT a US citizen, provide us with a copy of your green card (front & back), or if you are NOT a permanent resident provide us with your H-1 or L-1 visa. NOTE TO ALL No Income verification loans! If you have an interest in a no income verification loan, please call!
